In BlueDolphin it is possible to add questionnaires to an object type in the Admin module.
A questionnaire is a group of fields that add more details to objects of a certain object type (or types).
You have the option to assign a descriptive name to a set of fields and link the questionnaire to one or more object types.
Questionnaires are included to gather information for a particular reporting requirement.
You can generate reports for each individual field within a questionnaire.
Go to the Admin menu and click on Object questionnaires to get the screen below.
Here you will find a list of existing questionnaires. Click on a questionnaire in the list to view and edit its fields. Additionally, you have the option to add a new questionnaire by clicking the Add questionnaire button .
Create a new questionnaire
Click the Add questionnaire button to create a new questionnaire. The following screen appears:
To create a questionnaire, enter a name for the questionnaire and click OK to confirm the action.
Once the questionnaire has been created, it opens up directly so you can add questions to it.
Add a field to a questionnaire
Once you have created a new questionnaire or selected an existing questionnaire, you can add and/or remove fields. Choose Add field to add a new field to the questionnaire. You will get the following screen:
When adding a field, you need to fill in the generic field options in the above screen Add field. The below table explains the various options in the questionnaire field screen.

Once you select a certain type of field in general options, several additional options may be shown, depending on the type of the field.

If you want to display questions in a different order to your users, you can select the field in question with the mouse and drag it to the preferred position.
