Add a ? or any other obvious indication for the description to show up when filling out questionnaires

When you create a lot of questionnaires and send a guest link for someone to fill out, it is not obvious that there is a description available that helps how to fill out answer the question.
Only when you know that you can hover over the name with the mouse and wait for a couple of seconds the description will be shown (see below - the cursor unfortunately is not shown but it was hovering over “Application Alias”)

This (hopefully minor) change to the GUI would really help in getting better quality responses in the questionnaires. Examples from real experience:
- ‘key user satisfaction’ field (numeric field 1 to 10) being filled in wrong or not at all (as repondent try to type ‘good’)
- CIA scores ‘HIGH, MODERATE, LOW’ filled in wrongly as the guidelines for these (provided in the description) are not seen
